Sperm extraction is a specialized procedure used to obtain viable sperm directly from the male reproductive tract in cases where sperm are absent from the ejaculate due to obstruction, ejaculation disorders, or testicular dysfunction. This method is vital for achieving biological parenthood in men with azoospermia (no sperm in semen) or severe oligospermia (extremely low sperm count). The extracted sperm are used in assisted reproductive techniques such as Intracytoplasmic Sperm Injection (ICSI). There are several techniques for sperm extraction, including Percutaneous Epididymal Sperm Aspiration (PESA), Testicular Sperm Aspiration (TESA), Testicular Sperm Extraction (TESE), and Microdissection TESE (Micro-TESE). The choice of method depends on whether the infertility is obstructive (due to blockages in the reproductive tract) or non-obstructive (caused by testicular failure). In PESA, a fine needle is inserted into the epididymis to aspirate fluid containing sperm. This minimally invasive method is quick and often performed under local anesthesia. TESA and TESE involve retrieving sperm directly from testicular tissue using a needle or a small biopsy incision, respectively. Micro-TESE, a more advanced form, utilizes a surgical microscope to identify seminiferous tubules that are more likely to contain sperm, increasing success rates while minimizing tissue damage. The extracted sperm are examined under a microscope to identify motile and morphologically normal sperm for use in ICSI. Since sperm retrieved from these methods are typically few and immotile, they cannot be used for conventional IVF or IUI. Any excess viable sperm can be cryopreserved for future cycles, eliminating the need for repeated procedures. Sperm extraction is a relatively safe and effective option, though minor risks include testicular discomfort, infection, or hematoma formation. The procedure is particularly valuable for men with congenital bilateral absence of the vas deferens (CBAVD), post-vasectomy patients, or those with obstructive azoospermia due to trauma or infection. Overall, sperm extraction has transformed the treatment landscape for male infertility, allowing men who previously had no chance of producing sperm to become biological fathers.
