ICSI

Intracytoplasmic Sperm Injection (ICSI) is an advanced laboratory procedure used in conjunction with in vitro fertilization (IVF) to overcome severe male infertility or previous fertilization failures. The technique involves the direct injection of a single, morphologically normal sperm into the cytoplasm of a mature egg under high magnification using a specialized micromanipulator. ICSI was developed in the early 1990s and revolutionized the treatment of male infertility by allowing conception even in cases with extremely low sperm counts, poor motility, or structural sperm abnormalities. It is also beneficial for couples who have experienced failed fertilization in previous IVF cycles or are using surgically retrieved sperm. The procedure begins with standard IVF ovarian stimulation and egg retrieval. Meanwhile, the male partner provides a semen sample, or sperm is extracted surgically through procedures like testicular sperm extraction (TESE) or percutaneous epididymal sperm aspiration (PESA). The sperm are then examined, and a single motile sperm is selected using micromanipulation techniques. Under a microscope equipped with microneedles, the embryologist immobilizes the sperm and injects it directly into the egg’s cytoplasm. The fertilized egg is monitored for normal development, and successful embryos are cultured for 3 to 5 days before being transferred into the uterus. ICSI has a fertilization success rate exceeding 70%, significantly higher than conventional IVF for male factor infertility. It is particularly useful when sperm concentration is low or when sperm cannot penetrate the egg naturally. The embryos produced through ICSI develop and implant similarly to those from traditional IVF. Although ICSI is highly effective, it carries minor risks, including potential damage to the egg during injection and a slightly increased chance of chromosomal abnormalities due to the bypassing of natural selection mechanisms. Therefore, preimplantation genetic testing (PGT) may be recommended in some cases. Overall, ICSI represents a breakthrough in reproductive medicine, providing couples with a realistic chance of parenthood when natural or conventional fertilization methods are not viable.
