A Hysterosalpingogram (HSG) is a specialized diagnostic imaging procedure used to evaluate the shape and structure of the uterus and the patency (openness) of the fallopian tubes. It plays a crucial role in assessing female fertility and is often one of the first investigations recommended for women experiencing difficulty conceiving before starting IVF or other assisted reproductive treatments. During the procedure, a contrast dye is gently injected into the uterine cavity through the cervix using a thin catheter. As the dye fills the uterus and fallopian tubes, a series of X-ray images are taken. These images help the doctor visualize the internal uterine contour and track the movement of the dye through the tubes to determine whether they are open or blocked. An open fallopian tube allows the dye to spill freely into the pelvic cavity, indicating normal function. In contrast, a blocked tube prevents dye passage, suggesting conditions like tubal obstruction, scarring, or hydrosalpinx (fluid accumulation). The shape of the uterine cavity may also reveal structural abnormalities such as fibroids, polyps, congenital defects (like a septate uterus), or adhesions—all of which can affect implantation or increase the risk of miscarriage. The HSG is usually performed shortly after menstruation but before ovulation, to ensure the uterus is clear and the patient is not pregnant. Although it may cause mild cramping during the injection of dye, the discomfort is temporary and manageable with mild pain relievers. The entire procedure typically lasts less than 30 minutes. Beyond its diagnostic purpose, HSG can sometimes have therapeutic benefits. The passage of contrast dye can clear minor blockages in the fallopian tubes, potentially improving fertility. Many women have reported successful natural conception in the months following a normal HSG. In the context of IVF, HSG is essential in pre-treatment evaluation. It helps determine whether the tubes should be left intact or removed before IVF, especially if hydrosalpinx is present, as fluid from damaged tubes can leak into the uterus and impair embryo implantation. Understanding uterine and tubal anatomy beforehand allows fertility specialists to plan an effective and personalized IVF approach.
